  • Complete the Flu Vaccine Consent Form before you arrive and bring with you for entry into the Express Line. Forms will also be available at the administration site.
  • View CDC's 2008 Vaccine Information Statement for flu vaccine. VIS will also be available at the administration site.

Employees may also receive the flu vaccine at:

  • Employee Health Services' office, 57 Bee Street, Monday through Friday from 7:30am to 3:30pm, with faster service between 1:00pm and 3:00pm.
  • Beginning 10-17-08, Emergency Departments at 1 West, Charleston Memorial Hospital (CMH), and Chest Pain Center, everyday from 4:00am to 9:00am.

Managers may administer flu vaccine to their departments' employees. Fill out the Flu Vaccine Request Form and fax to 792-1200.

Remember, it takes approximately two weeks to build immunity, so it is recommended that employees receive the flu vaccine at least two weeks before holiday gatherings with family and friends.
