Current employees may access their immunization records online via My Records. Employees must have NetID account (formerly MNA) and password to access.
Immunization records of employees who came through our office prior to 2004 may not be available online. If your immunization record does not appear complete, please contact us so we may review your employee health file and enter missing information.
If you are not a current employee or if you need additional immunization information, print out the Authorization to Disclose Protected Health Information, fully complete, and fax to 792-1200. Records can be picked up in person or mailed.
- A fee for copies of protected health information will be charged. The fees to be charged are set by South Carolina law (SC 44-115-80): sixty-five cents per page for the first thirty pages and fifty cents per page for all other pages.
- Fees must be paid to Employee Health Services in advance before any records will be released.
- Records requests will be processed as quickly as possible. However, please allow THIRTY DAYS per South Carolina law (SC 44-115-80).
